Health Examination Certificates: Issuance and Validity Period

The health certificate issued by the Nurse's Office at KUAS is based on the results of the annual health checkup conducted in April of the relevant year. The certificate will be issued within the school year in which the medical examination was conducted, and is generally valid for three months. Issuance begins in early May after the regular medical checkup. Health checkup certificates can be issued at convenience stores using the Convenience Store Certificate Issuance Service.   • Applying for scholarships
  • Admission to graduate school
  • Transferring to another university
  • Applying for a part-time job
  • Procedures for away games and training camps for club activities, etc.

Important Points when Applying for a Certificate

For students transferring to another university

Some institutions require tests that are not provided as part of the standard KUAS health checkup, so please allow plenty of time to receive these tests. If the institution you will transfer to requires that you go to a national or public hospital or medical clinic, KUAS cannot conduct the checkup.

For athletes

In order to receive a health examination certificate, you must have taken the special physical examination for PE club members in addition to the regular physical examination held in April. For the group health certificate required of a club, please fill in the names of your club members and apply two weeks in advance.
